In order to receive your CIO certificate, you must fill out this form after you have completed the following courses:

ISTM 226,
ISTM 290 Project Management (formerly known as MGT 231),
ISTM 271,
ISTM 280,
ISTM 282,
ISTM 284,
ISTM 287,
ISTM 272, and
ISTM 274 (also formerly known as MGT 290 Survey of IT Technologies).

Note: Except as otherwise indicated, all "ISTM" courses above were formerly known as "MGT" courses with the same numeric designations. For example, "ISTM 280" was formerly called "MGT 280."
